Ulu Laukahi Project
The Ulu Laukahi Project is officially underway!
Steph David is the Lead Health Educator and she has completed the heath consultations for the 100 Ulu Laukahi Project participants.The Ulu Laukahi Project is designed for Native Hawaiians struggling with high blood pressure, high blood sugar levels, and/or weight. This year long free project has been made possible by the HMSA Foundation and Medtronics. The next phase is the fitness, nutrition, and well-being activities! We want to thank our community partners who we are collaborating with:
Kuleana Wellness Club, Waimea Yoga, and Five Mountain Fitness.
As additional funding becomes available we will add additional fitness vendors and activities to the program!
Sliding Fee Discount Program
KOKO is committed to providing services to all patients, and will make available discount services to those in financial need. All patients shall be charged without prejudice of their ability to pay. If unable to pay in full - KOKO has a Sliding Fee Discount Program. KOKO will also make efforts to cover expenses in a Pro Bono manner (waiving of fees) in cases where absolutely no financial resources are available.
KOKO has a Sliding Fee Discount Program and utilizes the “Annual Income Thresholds by Sliding Fee Discount Pay Class and Percent Poverty” as established in the 2017 Federal Poverty Guidelines to determine eligibility.
Please complete the Sliding Fee Discount Program application (Downloadable Forms Tab) and turn into the Front Desk at the Clinic. If you would like assistance in filling-out this form, pleas let us know, as our Clinical Case Manager will be happy to help you complete this application.
